Please be advised that due to the implementation of the strategy enabling Genexo Sp. z o.o. focus on our own portfolio of specialist products, as of October 31, 2019, we end our cooperation with Omron Healthcare as an authorized distributor of the Omron brand.

As of November 1, 2019, Procefar Sp. z o.o., taking over responsibility for commercial policy, promotion, marketing and distribution of products.

Omron Healthcare will be responsible for servicing the Omron brand as of November 1, 2019.

Providing data of the person making the report is necessary to accept the report. Only employees authorized to process personal data pursuant to the provisions of law and SciencePharma will receive your personal data. You have the right to access your data and correct it. Your personal data will be stored as long as the drug is authorized and for ten years after the marketing authorization has expired. You have the right to submit a complaint to the President of the Personal Data Protection Office if you determine that the processing of your personal data infringes on the GDPR provisions.

You have the right to submit a complaint to the President of the Personal Data Protection Office if you determine that the processing of your personal data infringes on the GDPR provisions.

INFORMATION FOR JOB CANDIDATES

As the personal data controller, Genexo Sp. z o.o. with its registered office in Warsaw, ul. Gen. Zajączka 26, hereby informs you that the candidate’s data is processed in order to conduct the recruitment process pursuant to the provisions of the Labor Code and the voluntary granted consent if the candidate provides more information than required by law. Each candidate applying for a job has the right to access their data, have it erased (unless the provisions of law stipulate otherwise) or corrected, to data portability as well as to object to further data processing. In the case of data outside the scope set forth in the Labor Code, processed pursuant to a voluntary consent, the candidate has the right to withdraw their consent, with no effect on the processing performed before the withdrawal. The candidate has the right to file a complaint to the President of the Personal Data Protection Office, should the data controller infringe on the requirements set forth in the personal data protection laws. As the personal data controller, Genexo Sp. z o.o. with its registered office in Warsaw, ul. Gen. Zajączka 26, hereby informs you that data is processed for the purpose of mail merge. The basis for processing your personal data is the controller’s obligation.

Each person has the right to access their data, have it erased (unless the provisions of law stipulate otherwise) or corrected, to data portability as well as to object to further data processing. In the case of data processed pursuant to a voluntary consent, the person has the right to withdraw their consent with no effect on the processing performed before the withdrawal. Each person has also the right to file a complaint to the President of the Personal Data Protection Office, should the data controller infringe on the requirements set forth in the personal data protection laws. The data is processed for the period of company’s existence. The data will be disclosed to Poczta Polska or courier companies.

Pursuant to Article 13 sections 1 and 2 of the Regulation (EU) 2016/679 of the European Parliament and of the Council
of 27 April 2016 on the protection of natural persons with regard to the processing of personal data

that contact data (e-mail address, telephone number) are collected for marketing purposes pursuant to a voluntary consent.

HOTLINE

As the personal data controller, Genexo Sp. z o.o. with its registered office in Warsaw, ul. Gen. Zajączka 26, hereby informs you that personal data is processed for the purpose of:

  • enabling contact with Genexo;
  • concluding an agreement with Genexo;
  • providing Genexo’s offer (after granting a separate consent to such action);
  • confirming the acceptance of a complaint and responding to it to the indicated e-mail address or by mail;
  • answering questions during contact;
  • improving the hotline quality of service.

The basis for processing your personal data is your consent granted through calling the hotline.

We process the following personal data:

  1. name and surname;
  2. telephone number from which the call was made;
  3. e-mail address (for contact or marketing purposes, after granting a separate consent);
  4. address to which mail is to be delivered;
  5. recorded calls;
  6. other data provided by you during the call.

Each person has the right to access their data, have it erased (unless the provisions of law stipulate otherwise) or corrected, to data portability as well as to object to further data processing. In the case of data processed pursuant to a voluntary consent, the person has the right to withdraw their consent with no effect on the processing performed before the withdrawal. The data is processed until the consent to process it is revoked. This does not apply to situations in which the consent for processing personal data was granted during the call. In such a situation, the call will be archived to confirm the consent to processing.

  • Call Center Online with its registered seat in Wrocław 50-950, ul. Wita Stwosza 16, for the purpose of operating the computer system;
  • NETLAB Paweł Majewski, as the supplier of IT services for Genexo;
  • courier companies and Poczta Polska, for the purpose of delivering mail;
  • ZTM MEDSERVICE, for the purpose of providing maintenance of medical devices;
  • pl, as the supplier of cloud services (i.e. servers).
